Pluggable Databases Opened in Restricted Mode After PSU Patching

Applying the October 2016 PSU to the 126.96.36.199 Container Database with PDBs in UPGRADE mode results in PDBs getting stuck in restricted mode.
PDB_PLUG_IN_VIOLATIONS contains this error message - PSU bundle patch 161018 (Database Patch Set Update : 188.8.131.52.160119 (21948354)): Installed in the PDB but not in the CDB. NONE bundle patch 0 (): APPLY with PENDING status WITH ERRORS in the PDB.
