One instance started dropping incidents and filled the file system.

The generated incident trace file shows:

DIA-48001: internal error code, arguments: [kgepop: no error frame to pop to], [], [], [], [], [], [], []
DIA-48001: internal error code, arguments: [dbgfcsGetRegisterDef:2], [33], [33], [], [], [], [], []

Database restarted without Application tier up and no new trace was being generated.
After the Application tier was reconnected errors resumed.
