Symptoms
On : 11.2.0.4 version, XML Database
After uprgade to 11.2.0.4 - xmltype.transform does not work as expected
ORA-31011: XML parsing failed
ORA-19202: Error occurred in XML processing
LPX-00234: namespace prefix "tns" is not declared
