RCONFIG Failed With oracle.ops.mgmt.cluster.ClusterException: Failed to create directory

Symptoms

When converting Single Instance Standby database to RAC standby database, the rconfig failed with below error.

$ <ORACLE_HOME>/bin/rconfig <ORACLE_HOME>assistants/rconfig/sampleXMLs/ConvertToRAC_ORCL.xml
Converting Database "ORCL" to Cluster Database. Target Oracle Home: /u01/app/oracle/product/11.2.0.4/dbhome_1. Database Role: PHYSICAL STANDBY.
Setting Data Files and Control Files
Adding Database Instances
Adding Redo Logs
Setting TEMP tablespace
Assigning UNDO tablespaces to instances
Adding Trace files
