Symptoms
Archiving stops working. The alert log shows the following:

ARC0: Error 19504 creating archivelog file '$1$DGA4:[TEST.ARCH]ARCH0001_91595.ARC'
ARC0: Archiving not possible: error count exceeded
ARC0: Failed to archive log# 1 seq# 91595
ARCH: Archival stopped, error occurred. Will continue retrying

ORA-16014: log 1 sequence# 91595 not archived, no available destinations
