"WARNING: Wrong OS disk s0 partition info" in Step 1 of BDA Mammoth V4.5/V4.6 OL6 on BDA Base Image 4.5.0-6

Symptoms

Step 1 of a BDA v4.5.0 or v4.6.0 Mammoth install on a Base Image of 4.5.0-6 fails as below with partitions issuing messaging about the "primary" partition name.  This messaging will be raised for Node 1 on Step 1 of install, but during the reboot step on the rest of the nodes.

Note the issue here is not the same as that reported on OL6 in: Step 8 of OL5 Mammoth V4.8 Upgrade Raises "Wrong OS disk s0 partition info : 1 ext3 raid 2 ext3 raid 3 linux-swap 4 ext3 ext3" Running Validation Tests (Doc ID 2279524.1) or on OL5 in: Step 8 of OL5 Mammoth V4.8 Upgrade Raises "Wrong OS disk s0 partition info : 1 ext3 raid 2 ext3 raid 3 linux-swap 4 ext3 ext3" Running Validation Tests (Doc ID 2279524.1).  Although the symptoms here look similar this note describes a different issue than in those notes.

The same can be seen in /root/BDA_REBOOT_WARNINGS and in bdachecksw/bdacheckcluster at the time of the Mammoth failure.  Therefore this will affect Step 1, any reboots during the upgrade, which will generate /root/BDA_REBOOT_WARNINGS, and the final cluster health checks.

To confirm the issue verify:

To Confirm the issue do the following on Node 1 as 'root' user.

1. Verify the Mammoth version is 4.5 or 4.6.

2. Verify the 4.5.0-6 base image.

Verify the 4.5.0-6 rpm is installed as below:

a) Confirm the rpm in /root/Extras/BDA.
