In a Grid Infrastructure (clustered) environment running on Windows Server 2012 R2, Create Database via DBCA (Database Configuration Assistant) fails reporting: 

Instance alert logs for both the partially created database (ORACLE_BASE\<db install userdiag\rdbms\<db name>\<db sid>\alert_<db sid>.log) as well as the relevant ASM instance (ORACLE_BASE\<db install user>\diag\asm\+asm\<asm sid>\trace\alert_<asm sid>.log) indicate that the database encounters ORA-1017 when it tries to connect to the ASMB0 process.
