How to identify the disk slot in ODA server when the failed disk / slot LED is also green

Context: OAK Stack

Disk showed as failed.
Removed the disk from the diskgroup, but left the disk physically in the ODA.
Ordered a new disk
Preparing to replace the disk, we were unable to identify the slot and disk location for replacement.

Reason: Disk was no longer visible using oak commands so unable to use Locate Disk command.
