Database And ASM Instance Ora-27300 OS System Dependent Operation Fork Failed With Status 11 (Doc ID 2331884.1)

Symptoms

On : 12.2.0.1 version,  see below errors in DB or ASM alert log. But could occur on lower version databases too.

When checking crs, listener status become INTERMEDIATE

ora.ASMNET1LSNR_ASM.lsnr
ONLINE INTERMEDIATE xxxxx STABLE

orachk, RDA HCVE, OSW, CVU do not report relevant issues.

Rebooting the node could be a workaround for this issue, but after the node is up for some time issue will reoccur.

 

In one another case,  customer also reports

During cluster (2-nodes) startup the CHA daemon failed on the second node.

found following output in ochadOUT.trc:
