Grid Infrastructure Upgrade : Runinstaller Fails with Error "INS-41706 Only subset of nodes selected for upgrade"

Symptoms

 While upgrading Oracle Clusterware from 10gR2 to 11gR2,runInstaller displays 4 nodes where as the cluster is of 2 nodes only.

runInstaller also gives error that ssh is not configured between all nodes. The ASM upgrade checkbox gets hidden.

The opatch lsinventory shows 4 nodes where as cluster is actually having 2 nodes.
