Grid Infrastructure Upgrade Fails as Incorrect Locations Are Picked Up in

ODA upgrade to failed during grid patching:

There is no directory as: /u01/app/12.2.0/grid/perl/bin/ exist in the server

 ERROR : Ran '/bin/su grid -c "/opt/oracle/oak/onecmd/tmp/"' and it returned code(127) and output is:
/u01/app/ line 48: /u01/app/12.2.0/grid/perl/bin/perl: No such file or directory

: ERROR : /bin/su grid -c "/opt/oracle/oak/onecmd/tmp/" did not complete successfully. Exit code 127 #Step -1#

The can happen on node0 (first Node) cause both nodes upgrade failed or node1(second node) only.  We have different solution for these two situation.
