ODA: Grid Upgrade To 12.2.1.2 failed when run rootupgrade.sh related to CLSRSC-540 (Doc ID 2361621.1)

This can happen on ODA HA and S/M/L platform both when upgrade from 12.1 -> 12.2

Symptoms

Updating from 12.1.2.12.0 to 12.2.1.2.

This ODA upgrade problem may be generic to 12.2.x with the below mentioned error messages.
