Symptoms
Multi-node global transaction running in a PDB (in RAC) ends up in prepared state SCO|COL|DEAD but without an entry in the pending transaction tables.
i.e The transaction shows as 'PREPARED' on table x$ktuxe but there are no entries associated to it in dba_2pc_pending view.
The transaction also cannot be cleaned manually by inserting dummy entries in the pending transactions and pending sessions tables and executing a commit force
or rollback force of it and then purging the transaction.
