12.2: Grid Infrastructure Upgrade Fail with PRCR-1001 : Resource ora.qosmserver does not exist (Doc ID 2386605.1)

Symptoms

While performing Grid Infrastructure (GI) upgrade from 12.1.0.2 to 12.2.0.1, rootupgrade.sh result below error

rootcrs<datetime>.log inside <oracle base>/crsdata/<hostname>/crsconfig: 

2018/04/15 11:36:03 CLSRSC-595: Executing upgrade step 18 of 19: 'UpgradeNode'.

CRS-1115: Oracle Clusterware has already been upgraded.

2018/04/15 11:36:10 CLSRSC-478: Setting Oracle Clusterware active version on the last node to be upgraded

2018/04/15 11:36:10 CLSRSC-482: Running command: '/u01/app/12.2.0.1/grid/bin/crsctl set crs activeversion -force'

CRS-1115: Oracle Clusterware has already been upgraded.

2018/04/15 11:37:10 CLSRSC-479: Successfully set Oracle Clusterware active version

2018/04/15 11:37:14 CLSRSC-476: Finishing upgrade of resource types

PRCR-1001 : Resource ora.qosmserver does not exist

2018/04/15 11:37:37 CLSRSC-180: An error occurred while executing the command '/u01/app/12.2.0.1/grid/bin/srvctl upgrade model -s 12.1.0.2.0 -d 12.2.0.1.0 -p last'

2018/04/15 11:37:37 CLSRSC-482: Running command: 'srvctl upgrade model -s 12.1.0.2.0 -d 12.2.0.1.0 -p last'============>>>>>>>>>>>>>>>>>>>>>>>>

2018/04/15 11:37:37 CLSRSC-284: Failed to perform last node tasks for cluster modeling upgrade

The command '/u01/app/12.2.0.1/grid/perl/bin/perl -I/u01/app/12.2.0.1/grid/perl/lib -I/u01/app/12.2.0.1/grid/crs/install /u01/app/12.2.0.1/grid/crs/install/rootcrs.pl -upgrade -force' execution failed
