Cluster Health Monitor Diagnostics Are Not Collected (via TFACTL Diagcollect) and ora.crf Cannot be Started (Doc ID 2395118.1)

Symptoms

Cluster Health Monitor (CHM) diagnostics were not becoming collected via 'tfactl diagcollect'. What appeared to be the case is that sysmond is not fully coming up and therefore, not starting ologgerd as it should.  Additionally, the ora.crf resource could not be started.
