Cluster Expansion on BDA (Big Data Appliance) Fails on Step 5 During MySQL Setup (Doc ID 2435822.1)

Symptoms

Cluster expansion "./mammoth -e" fails on Step 5 (Setup MySQL) with errors like below for the first new nodes:

ERROR: Some puppet agents are not reachable
ERROR: Puppet agent on node bdanode0x is not reachable
ERROR: Cannot continue with install #Step 5#
INFO: Time spent in step 5 SetupMySQL is 2086 seconds.
