Symptoms

One or more errors of the following form appear in the alert log during startup of an 11.2.0.3 database:

ORA-07445: exception encountered: core dump [sltsimr()] [SIGSEGV] [ADDR:0x0] [PC:0x107344400] [Address not mapped to object] []
ORA-07445: exception encountered: core dump [spini()+152] [SIGSEGV] [ADDR:0x10C000] [PC:0x104E24AF8] [Address not mapped to object] []
ORA-07445: exception encountered: core dump [dbgeFreeIncIdCache()+20] [SIGBUS] [ADDR:0x108815934] [PC:0x108815934] [Invalid address alignment] []
ORA-00600: internal error code, arguments: [902], [0], [1], [], [], [], [], [], [], [], [], []


In addition, there are failed database startups due to critical background processes not starting, e.g.

Sun Jul 29 04:27:36 2018
Process LGWR died, see its trace file
USER (ospid: 22753): terminating the instance due to error 443
Instance terminated by USER, pid = 22753

along with many messages concerning background processes not starting, e.g.

ORA-00445: background process "W000" did not start after 120 seconds
ORA-00445: background process "m001" did not start after 120 seconds
