Symptoms

After deploying agents and adding MySQL instances to Oracle Enterprise Manager the main page for the MySQL instances shows a spinning box and the page stays in loading state.

Checking the java script console while that page is loading may show a missing library javascript errors for /em/emx/js/libs/require/require-debug.js
