PDB_PLUG_IN_VIOLATIONS Pending Actions are not Cleared Even After the Actions have been completed (Doc ID 2569782.1)

Symptoms

There are errors or pending actions in PDB_PLUG_IN_VIOLATIONS.

After fixing all pending actions, they are not removed from  PDB_PLUG_IN_VIOLATIONS and PDB still opens in restricted mode.
