AutoUpgrade fails in POSTFIXUP phase of RAC DB Upgrade (Doc ID 2575477.1)

Symptoms

When trying to upgrade a 12c RAC Database to 19c using the AutoUpgrade tool, the following error is reported in the alert log / trace file:

ORA-00603: ORACLE server session terminated by fatal error
ORA-01092: ORACLE instance terminated. Disconnection forced
ORA-39701: database must be mounted EXCLUSIVE for UPGRADE or DOWNGRADE
###


This happened in stage "POSTFIXUPS" after the "UPGRADE" stage.
