AFDLIB IO Failed errors in ASM alert log (Doc ID 2595760.1)

Symptoms

 Create diskgroup reported below messages in ASM alert log.

SQL> CREATE DISKGROUP TEST EXTERNAL REDUNDANCY DISK 'AFD:TEST1' SIZE 9496M
ATTRIBUTE 'compatible.asm'='12.2.0.1','compatible.advm'='12.2.0.1','au_size'='4M'

NOTE: Assigning number (4,0) to disk (AFD:TEST1)

2019-10-02T11:05:56.326256+08:00
NOTE: Disk TEST1 in mode 0x7f marked for de-assignment
SUCCESS: diskgroup TEST was created

2019-10-02T11:06:03.640318+08:00
SUCCESS: diskgroup TEST was mounted
2019-10-02T11:06:03.716690+08:00
NOTE: diskgroup resource ora.TEST.dg is online

 Drop diskgroup reported below messages in ASM alert log 

SQL> drop diskgroup TEST including contents;

NOTE: erasing header (replicated) on grp 4 disk TEST1
NOTE: erasing header on grp 4 disk TEST1
SUCCESS: diskgroup TEST was dropped

NOTE: Disk TEST1 in mode 0x7f marked for de-assignment
NOTE: cache deleting context for group TEST 4/0xd42e344e

NOTE: diskgroup resource ora.TEST.dg is dropped

SUCCESS: drop diskgroup TEST including contents

 

System Admin removed disks from OS level.

Once the disk removed from OS, it is started reporting below error in ASM alert log:

AFDLIB FAIL: failed to get devnum for /dev/rdsk/ora_disks/TEST_DG_001          
AFDLIB IO Failed @ 0xffffffff7cfebe88: issync=0 que=wait err=5 sts=0x2e dsk=0x2808e90eaf fst=0
