Upgrade to Mammoth 4.14 OL6 Reboot Fails-Servers Are Hung at the Grub Menu (Doc ID 2598813.1)

Symptoms

When upgrading to BDA 4.14 the symptom here is that no servers are able to reboot at the Mammoth reboot step. From the ILOM console all server reboots are hung at the grub menu. The grub menu looks correct.

The examples here are based on a BDA 4.14 OL6 upgrade where the default BDA 4.14 kernel is "4.1.12-124.18".

Using BDA 4.14 OL6 as an example, the grub menu in the ILOM could correctly show four entries for the BDA V4.14 OL6 "4.1.12-124.28.1" kernel, four for the rescue mode kernel and four for the previous kernel.

Entering the grub command line shell by entering 'c' from the Grub menu screen and following the steps in, How to Identify Missing BDA Kernel Files from the Grub Shell On OL6 (Doc ID 2598819.1), identifies a missing initramfs kernel file.  For example on BDA 4.14 OL6 the kernel file, initramfs-4.1.12-124.28.1.el6uek.x86_64.img, is missing.

After identifying the missing file and exiting the grub shell, it is possible to scroll down and boot into a pre-BDA 4.14 kernel.
