Oracle Database Appliance configuration fails at: "Storage discovery" and "Discovering DiskGroup" steps failed on trying to deploy ODA configuration after reimaging.

Errors seen in corresponding dcs-agent.log:

DCS-10001:Internal Error Encountered: Fail To discover disk groups.

Errors can be similar to or any of the following

•  ORA-15030: diskgroup name ".." "is in use by another diskgroup" ;
•  ORA-15018 "diskgroup cannot be created" ; Delete Reference
•  [FATAL] [INS-30530] Following Specified Disks Have Invalid Header Status:
•  [INS-30508] Invalid ASM Disks [FATAL]
•  [INS-30516] Please specify unique disk groups
•  DCS-10001:Internal error encountered: Fail to run root scripts;
•  ORA-15030: diskgroup name "DATA" is in use by another diskgroup
