ORA-12514 with connections but Database is mounted and open and LOCAL_LISTENER is set correctly (Doc ID 2621090.1)

Symptoms

Possibly after a reboot, connections are failing through the Listener:
ORA-12514

The Database is mounted and open and the LOCAL_LISTENER (Database) Parameter is correctly set to the Listener TCP address:
LOCAL_LISTENER     '(address = (protocol = tcp)(host = <listener_hostname>)(port = <listener_port#>))'

Also, IPv6 is DISABLED on this Server.

Changes

A restart of the Server or Listener after some operating system security  updates / installations.
