Repeated Message "skvol Init Failed=1“” Found In ASM Trace File (Doc ID 2682229.1)

Symptoms

1. Found repeated message in ASM trace file (as below), which extend the file to huge size.

skvol init failed=1
skvol init failed=1
skvol init failed=1
skvol init failed=1
skvol init failed=1
skvol init failed=1

......

......

 2. Found repeated "parse error" in ASM alert log.  The SQL is to query dba_users which does not exist in ASM instance.
