Symptoms

DBUA fails with error Failed to start the database in upgrade mode.

In the alertlog the following error appears:

WARNING: failed to register ASMB with ASM instance

ORA-15001: diskgroup "DATA" does not exist or is not mounted
ORA-01017: invalid username/password; logon denied
