Opacthauto fails with error "PRCR-1172 : Failed to execute "srvmhelper" for -getTargetHubSize"

Symptoms
July 2020 PSU patch application fails with the below error.

Opatch auto precheck error - OPATCHAUTO-72050
on Doing precheck to apply July Bundle patch for 12c DB home and 18c Grid home. we get the following error.
OPATCHAUTO-72050: System instance creation failed.
OPATCHAUTO-72050: Failed while retrieving system information.
OPATCHAUTO-72050: Please check log file for more details.
In both case systemconfig log shows below errror.
com.oracle.glcm.patch.auto.db.integration.model.productsupport.topology.TopologyCreator - Not able to retrieve system instance details :: Unable to determine if "<GI_HOME_PATH>" is a shared oracle home.

Verification cannot proceed
The result of cluvfy command does not contain OVERALL_STATUS String.
