ORA-12152 Errors when DRCP in Use (Doc ID 2735203.1)

Symptoms

Database transactions are failing, and a series of ORA-12152 errors in either the database alert log or the DRCP trace files:
opitsk: network error occurred while two-task server trying tosend break;
error code = 12152
Pool server terminating with error=12152
OPIRIP: Uncaught error 12152. Error stack:
ORA-12152: TNS:unable to send break message
ORA-12152: TNS:unable to send break message
