INSERT fails with ORA-22275, ORA-31159: XML DB is in an invalid state (Doc ID 2808959.1)

Symptoms

After upgrading a database, where Oracle Label Security (OLS) is being used, from 12.2 to 19c, updates by users through the front end tool generates a pop-up window with the error:

ORA-06502: PL/SQL: numeric or value error: invalid LOB locator specified: ORA-22275


While an error is logged in the alert.log:

The trace file shows:


Connecting directly to the database using SQLPLUS and inserting a dummy row to the same table then rolling it back works around the issue temporarily.
