Symptoms

 ORA-600 [kffbAddBlk04] triggered in the ASM Instance while adding/creating files in the ASM Diskgroup.

 

 

The ORA-600[kffbAddBlk04] error indicates ASM metadata corruption.

This assert means that ASM needed to get a new block either in ASM File Directory (ASM File# 1) or ASM alias directory (ASM File# 6) for an ASM file allocation.
ASM free block Manager (List-head) reported that the block was free, but the block information shows it is already allocated.
This is a fatal inconsistent state of ASM Metadata.
