Symptoms
The problem manifested itself with a combination of errors in the alert log:
- continuous "ORA-12850: Could not allocate slaves on all specified instances: needed, allocated" errors for many hours
- followed by"kkjcre1p: unable to spawn jobq slave process," and "CLMN: clean deferred state objects - failed messages,"
- followed by high waits in LMD0 for 'latch: shared pool' for XX secs";
- finally, the instance terminated with ORA-481: "LMON process terminated with error."
