Symptoms
One of the quorum disks for an ASM diskgroup is reported missing when querying v$asm_disk.
For example, issuing "select name,header_status,mount_status,path from v$asm_disk" shows
SQL> select name,header_status,mount_status,path from v$asm_disk;
GROUP_NUMBER MOUNT_S HEADER_STATU NAME LABEL
------------ ------- ------------ ------------------------------ -------------------------------
PATH

1 MISSING UNKNOWN QD_DATA_<DB node1>_MG
