Fast Refresh Sometimes Does Not Refresh Materialized View (Doc ID 2881760.1)

Symptoms

On very rare occassions, fast refresh of a materialized view does not refresh correctly.

Master table's changes are correctly inserted into the
materialized view log table (mlog$_<master_table_name>)
but they are not reflected to the materialized view during fast refresh.
