Symptoms

After failover a group, database resource cannot start due to ORA-28002.

Application log

FS-10318: Oracle Fail Safe failed to online resource <DB>
ORA-01034: ORACLE not available
ORA-27101: shared memory realm does not exist

ORA-28002: the password will expire within 7 days
