Symptoms

While creating datafiles (via rman restore, file create as part new database creation etc) on ASM diskgroup which is hosted on Exadata Extended/XT storage cells, rman/database may report below errors:

kcfis: Exadata error: Device 'o/Cell_PrivateIP1;Cell_PrivateIP2//box/predicate' error code 212 (Offload library initialization failed) app_type 5 dbopen 'no'
ORA-27603: Cell storage I/O error, I/O failed on disk o/Cell_PrivateIP1;Cell_PrivateIP2//box/predicate//box/predicate at offset 0 for data length 0
ORA-27626: Exadata error: 212 (Offload library initialization failed)
kcflfi:ASM conf for fob=0x22c00d46b0 file=+DATAXT/DB_NAME/DATAFILE/appl_data.907.1106388201 changed to non-SAGE

File creation or restore operation successfully completes without any other issues. But, just that, database is prints above errors in instance trace/alert log.

Changes

Using Exadata Extended Storage (available starting X8-2) for archived/cold data which may be needed for regulatory reasons amongst others.
Here, in this case, +DATAXT is ASM disk group solely having disks on Extended Storage nodes.
Issue is not reproducible with disk groups hosted on non-Extended ('normal') Storage nodes.

"enableSmartStorage" attribute for XT cells is already set to FALSE

XTCell1: XTCell1 online FALSE
XTCell2: XTCell2 online FALSE
XTCell3: XTCell3 online FALSE
XTCell4: XTCell4 online FALSE
