Symptoms

Rootupgrade.sh failed to upgrade voting files while upgrading from 12.1 to 19c . 

2022/09/20 01:47:01 CLSRSC-4003: Successfully patched Oracle Trace File Analyzer (TFA) Collector.
CRS-8503: Oracle Clusterware process CSSVFUPGD with operating system process ID 20512848 experienced fatal signal or exception code 4.
Failure with signal 6 from command: /u01/app/19.0.0/grid/bin/cssvfupgd
Upgrade of voting files failed
2022/09/20 01:47:07 CLSRSC-243: Failed to upgrade the voting disks for Cluster Synchronization Service
Died at /u01/app/19.0.0/grid/crs/install/crsupgrade.pm line 4010.
