Symptoms

RMAN Backup using  Commvault Failing:
RMAN-03002: failure of backup plus archivelog command at 
RMAN-03009: failure of backup command on ORA_SBT_TAPE_4 channel at 
RMAN-10038: database session for channel ORA_SBT_TAPE_4 terminated unexpectedly
