Symptoms
It is only after a switchover and occurs in RAC and Data Guard configurations.
"shutdown" a new primary instance after Switchover crashes another instance other than "shutdown":
1) Switchover at standby to primary.
2) A instance "shutdown" in new primary.
3) RSnn hit ORA-600 in another instance while reconfigraion.
