Windows - ORA-15081: Failed To Submit An I/O Operation To A Disk - ORA-15025: could not open disk O/S-Error: (OS 2) (Doc ID 2954469.1)

Symptoms

No I/O errors on ASM but Database receiving error "ORA-15081: failed to submit an I/O operation to a disk" on only one of the RAC nodes

Changes

A hard reboot could caused the loss.

Frequently the system is rebooting as can be seen in the System Event Viewer logs.

To access the Microsoft Windows Event Logs (Application, System and Security) via the Control Panel > Administrative tools > Event Viewer > Right click on the appropriate log in Event Viewer and choose 'Save Log File As' txt format.
