Database I/O Errors And Database Hangs On Shutdown (Doc ID 299725.1)

Symptoms

 

1) Database shutdown hanging.
2) Oracle defunct porcesses or zombies are showing up and left on machine.
3) Sporadic I/O errors reported in the database alert log.

ORA-00604: error occurred at recursive SQL level 1
ORA-01115: IO error reading block from file 1 (block # 653)
ORA-01110: data file 1: '/u5/oradata/<SID>/system01.dbf'
ORA-27091: skgfqio: unable to queue I/O
ORA-27072: skgfdisp: I/O error
SVR4 Error: 12: Not enough space
Additional information: 638
