ASM Disk Group Incorrectly Dismounted When One Disk Offlined After I/O Error (Doc ID 3022589.1)

Node 1 OS messages log shows that Disk controller fatal error happened.
The disk group is with normal redundancy with 4 failgroup: failgroup 1 with 8 disks, failgroup 2 with 8 disks, failgroup 3 is a quorum failgroup with one disk, failgroup 4 is a quorum failgroup with one disk.
This I/O error affected one normal failure group and one quorum failgroup.

Disk controller fatal error happened.
