DB Instance not starting using SRVCTL on GI fresh Installation

Symptoms
When trying to start DB instance using SRVCTL below error messages are observed:
PRCR-1079 : Failed to start resource <DB_NAME>
CRS-5017: The resource action "<DB_NAME> start" encountered the following error:
ORA-03113: end-of-file on communication channel
Process ID: 3636750
Session ID: 237 Serial number: 49128
. For details refer to "(:CLSN00107:)" in "<ORACLE_BASE>/crs/<HOSTNAME>/crs/trace/ohasd_oraagent_grid.trc".
CRS-2674: Start of '<DB_NAME>' on <HOSTNAME>' failed
