Symptoms


When starting database instance using srvctl utility or sqlplus, database Instance fails to start with errors ORA-15032 ORA-15017 ORA-15040


srvctl start instance -d <DB_UNIQUE_NAME> -i <INSTANCE_NAME>


PRCR-1013 : Failed to start resource ora.<DB_UNIQUE_NAME>.db
PRCR-1064 : Failed to start resource ora.<DB_UNIQUE_NAME>.db on node <NODE1>
CRS-5017: The resource action "ora.<DISKGROUP_NAME>.dg start" encountered the following error:
ORA-15032: not all alterations performed
ORA-15017: diskgroup "DISKGROUP_NAME" cannot be mounted
ORA-15040: diskgroup is incomplete
