Symptoms
The following error may occur while applying one-off patches using OPatch
INFO:OPatch was able to restore your system. Look at log file and timestamp of each file to make
sure your system is in the state prior to applying the patch.

INFO:1) OUI-67124:Inventory check failed: Patch ID 123456 is NOT registered in Oracle Home
inventory.

This error can occur even while using the latest version of OPatch and while looking at the apply actions log, the patch may be getting applied successfully but failing to update the inventory and hence also failing at verification phase.
