Symptoms
Oracle recommends that the Database Configuration Assistant (DBCA) is used to add additional instances to existing RAC databases. However, there may be times when an instance will be added manually. In those cases, it is necessary to take the value for the CLUSTER_DATABASE_INSTANCES parameter into account. Failing to do so will result in failure to mount the new instance and the occurrence of the following error in that instance's alert.log:
