Receiving the following error in Enterprise Manager logs:
ORA-07445: exception encountered: core dump [ACCESS_VIOLATION] [kghfrf+154] [PC:
0x4F12EC2] [ADDR:0x1C56E69C] [UNABLE_TO_READ] ~
Current SQL statement for this session shows:

O/S info shows program: OMS
The problem is occurring during the cleanup of a shared server process that is already marked dead. It has problems freeing memory associated with a hash table, probably due to a memory corruption.
Looks like forcing the OMS to use dedicated server connections rather than shared servers (MTS) prevents the problem from occurring.
