ORA-904 And ORA-7445 [KKTTREX()+1524] When Performing Migration to 11g After Startup Migrate (Doc ID 462790.1)

Symptoms

When attempting to perform a migration from 10.2 to 11g, an ORA-904 and ORA-7445 [kkttrex()+1524]  are encountered:

The alert.log shows:
ORA-00904: "ACTIONCOLNO": invalid identifier
Exception [type: SIGSEGV, Address not mapped to object] [ADDR:0x40] [PC:0x8939E3E, kkttrex()+1524]
ORA-07445: exception encountered: core dump [kkttrex()+1524] [SIGSEGV] [ADDR:0x40] [PC:0x8939E3E] [Address not mapped to object] []

Stack trace is similar to the following:
