ORA-7445[QMRGSN], Ora-7445 [Qmtremoveref] While creating a new database (Doc ID 466502.1)

Symptoms

Alert.log shows the following error being generated while creating a new database

Exception [type: SIGSEGV, Address not mapped to object] [ADDR:0x8] [PC:0xA553A7D, qmtRemoveRef.()+87]
Errors in file /oracle/app/diag/rdbms/orcl1106/orcl1106/trace/orcl1106_ora_5613.trc (incident=11244):
ORA-07445: exception encountered: core dump [qmtRemoveRef.()+87] [SIGSEGV] [ADDR:0x8] [PC:0xA553A7D] [Address not mapped to object] []
Incident details in: /oracle/app/diag/rdbms/orcl1106/orcl1106/incident/incdir_11244/orcl1106_ora_5613_i11244.trc

Trace file shows that the Current SQL Statement is

Reviewing the alert.log for previous errors shows numerous occurences of
ORA-00600: internal error code, arguments: [qmrgsn], [4278370576], [], [], [], []

The following parameter is also set as follows
db_securefile = "ALWAYS"
