This problem can occur on any platform.
1. During a bulk insert job, the following grouping of errors are seen (the ORA-600  may or may not be seen in the grouping):
ORA-00600: internal error code, arguments: 
ORA-07445: exception encountered: core dump [ptdadh()+56][SIGSEGV] [Invalid permissions for mapped object]
ORA-04030: out of process memory when trying to allocate 1202072 bytes (PLS non-lib hp,DARWIN)
2. The failing SQL is an insert statement.
3. A bulk insert job is being run with save exceptions.
4. The insert job is either long running or is inserting into a table with an extremely large number of rows.
