Cannot Start Instance Using Srvctl, No Info in Imon Logs

When trying to start an instance using srvctl, the instance does not start.  The srvctl start command fails with CRS-0215.

$ srvctl start instance -d MYDB -i MYDB1
Attempting to start `ora.MYDB.MYDB1.inst` on member `myserver01`
`ora.MYDB.MYDB1.inst` on member `myserver01` has experienced an unrecoverable failure.
Human intervention required to resume its availability.
CRS-0215: Could not start resource 'ora.MYDB.MYDB1.inst'. 

Diagnostic analysis:

* The instance can be started with no errors by sql*plus.
* Tracing the srvctl command shows that the crs_start command fails with status 115.
* No information is recorded in the imon logs.

* crsd.log show the instance can't be stopped either after the instance start failure and set the instance state in UNKNOWN


Permissions and/or ownership to the imon logs under $ORACLE_HOME/log/<host>/racg have changed (although the DBA may not be aware of this change).
